The Clinical Observations of the Pediatric Surgical Patient


Abstract
Many advances have been achieved in the field of pediatric surgery lately due to improvement of anesthesia, transfusion, electrolyte & fluid therapy, surgical technique and parenteral nutrition. A clinical review was made of 1073 pediatric surgical patient under 15 years of age admitted at department of surgery, Chung-Ang university hospital from June 1968 to March
1980.
The result were as following,
1. Classification of disease
Hernia 250cases(23.3%)
Appendicitis 226cases(21.1%)
Burn 177cases(16.5%)
Intussusception 164cases(15.3%)
Mass 85cases(7.9%)
Abscess 59cases(5.2% )
Congenital anomalies 48cases(4.5%)
Miscellaneous 67cases(6.2%)
2. Among total cases, the most common age group was 11 years to 15 years of age and followed by 6~10 yrs, 3~5 yrs, 1~2 yrs, in order of frequency.
3. The ratio of male to female of total cases were 2.2 : 1; 6.6 : 1 in hernia, 1.7 : 1 in appendicitis, 1. 6 : 1 in burn, 2.3 : 1 in intussusception, 1.4 : 1 in mass, 1. 1 : 1 in abscess, 4.3 : 1 in congenital anomalies and 1. 9 : 1 in miscellaneous. The hernia and congenital anomalies were predominant in male.
4. The most common type of hernia was indirect inguinal hernia(97.6%), others . were diaphragmatic hernia(0.8%), umbilical hernia(0.8%), incisional hernia(0.4%) and internal hernia(0.4%).
5. The appendicitis was 226 cases and 44 cases(19.5%) were perforated.
6. The burn was 177 cases. The most common cause was scalding(89.3%) and the largest
nuumbers were occurred from 3 to 5 years of age(40.7%).
7. The intussusception was 164 cases and the most common type was ileocecal(59.2%). 8. The mass was 85 cases and tuberculous lymphadenitis(23.3%) was the most common.
9. The abscess was 56 cases and carbuncle & furuncle(80.3%) were the most common. 10. The congenital anomalies were 48 cases, there were imperforate anus(25.0%), conge
nital megacolon(22.9%), congenital hypertrophic pyloric stenosis & duodenal atresia(22.
9%), liver & biliary disease(12.5%), esophageal atresia with TEF(6.3%), congenital
spherocytosis(6.3%), gastroschisis (2. 1%) and omphalomesenteric fistula(2.1%).
11. The miscellaneous were 67 cases including trauma, peritonitis, intestinal obstruction,
anal fistula, peptic ulcer and bleeding, foreign body & bezoa, external hemorrhoid.
12. The overall mortality rate was 2.1%. The burn was accompanied with the highest
mortality (7. 3%) due to sepsis, asphyxia, electrolyte imbalance. The neonatal mortality
rate was 23.8% mainly due to respiratory failure and sepsis.
